There are certain guidelines in place that help nurses understand the best way to perform a procedure. When weaning a patient off the tracheostomy tube, the best way to wean off the patient is to plug the opening of the tube for about 5-20 minutes. In the mean time you need to monitor the patient’s respiratory status.
This will help make sure that the patient is not going into distress. Plugging for shorter intervals of time will be inadequate to produce a response. However, blocking the tube for more than 20 minutes increases the risk of respiratory distress which can be difficult for the patient to recover from.
5 to 20 minutes.-rationale: initially, the nurse should plug the opening in the tracheostomy tube for 5 to 20 minutes, then gradually lengthen this interval according to the clients respiratory status. a client who doesnt require continuous mechanical ventilation already is breathing without assistance, at least for short periods; therefore, plugging the opening of the tube for only 15 to 60 seconds wouldnt be long enough to reveal the clients true tolerance to the procedure. plugging the opening for more than 20 minutes would increase the risk of acute respiratory distress because the client requires an adjustment period to start breathing normally.client needs category: physiological integrityclient needs subcategory: physiological adaptationcognitive level: applicationreference: smeltzer, s.c., et al. brunner & suddarths textbook of medical-surgical nursing, 11th ed. philadelphia: lippincott williams & wilkins, 2008, p. 754.
