During the nursing process steps, all the options are involved and are in a step by step process, which includes assessment, planning, implementation, and evaluation.

Evaluation can be defined as the process in which a health care provider implements care, and it is reassessed or evaluated to be sure if the care provided is effective for the patient /client.

It is advisable to evaluate the care provided for a patient/client daily, depending on the severity of the patient/client condition.

During the process of evaluation or reassessment, it gives the health care provider reasons why the care provided should be continued or even stop especially when the patient/client is having the side effect of the care provided.

From time to time, a nurse will need to change a client’s care plan. This may be because the client is improving faster than expected. It could also be because the client’s prognosis has changed. The client may have made a turn for the worst. If this is the case, the nurse will need to change the care plan according to the doctor’s recommendations.

When the client enters the hospital, there are steps that the nurse takes. During this step, it is called the evaluation. The nurse must evaluate what steps to take to make the client feel better. This might include administering medication or requesting that the client undergo certain tests to determine the problem and then administer the treatment.
